Immunizations

 

All DHS/FEMA/NDMS personnel are required to comply with the immunization requirements by July 1, 2005. As many of these vaccinations are in series and can take up to six months to complete, you will need to start your needed vaccinations soon. FEMA will not allow anyone to deploy without the required immunizations.

 

Below is a list of the required vaccinations.

 

You may have the vaccinations at your family physician, your county Health Department or a Federal Occupational Health Clinic (FOH). The private practitioner will be the most costly route. The County Health Department is a significant savings, the Federal Occupation Health Clinic will bill FEMA.

 

A list of all FOH clinics is at the bottom of this message. If you choose to go to one of our FOH clinics, the clinic must be called in advance and an appointment must be set up - this will allow ample time for the clinic to receive the vaccines you will need.  Please allow roughly two weeks lead time to allow FOH to schedule the immunization clinic at a mutually convenient time. Note that FOH services are normally available only during weekdays, not on weekends.

NDMS personnel will need to provide official NDMS identification prior to receiving immunizations at the FOH clinic and have your PHS-731 with you. 

 

Immunization options are your choice.  If you choose to go to a FOH clinic, it is important that you inform the nurse that you are with FEMA - NDMS so that they may track for billing.

 

If you choose to use your private practitioner or the County Health Department, the cost of the immunizations is your responsibility. FEMA will not reimburse for these expenses.

 

Your immunization record must be documented on a Form PHS-731 (to follow). In the near future you will receive a form PHS-731 in the mail. 
 

You cannot be deployed without the completed PHS-731.

 

For the childhood diseases (Chickenpox, Measles, Mumps, Rubella, Polio) you will need to shown verifiable proof (physician’s report) of contracting the disease (Mom or dad told you that you had it doesn’t count), a verifiable history of immunization, a reactive blood titer or be re-vaccinated.

 

If you have a reason for not receiving any of the required immunizations, such as an allergy to the ingredients, you may request an Immunization Waiver Request form (FEMA form 90-137). The waiver must be signed-off by your Team Commander and may preclude you from future deployments.
